Mine where two died listed as pattern violator

The federal Mine Safety and Health Administration on Thursday designated Pocahontas Coal Co.'s Affinity Mine in Raleigh County, where two men died within two weeks of each other in February, as a pattern violator, meaning it repeatedly broke federal health and safety regulations.
